If I do not answer when you call:  Leave me all of your information I will need to call you back: full name, time to call you back, date you are requesting and location, In or Out call. 

At a hotel:  If you don't have a personal phone please make sure you leave me your hotel room number with your first & last name plus all the info above. When I call back as part of verification I will ask for you by name instead of room number to verify your room.  

(Hotel  policies:)  you have to know the parties full name in the room before they connect you to the room. If your room is under a business/company name then leave me your name and the business/company name room is under.

When you call to book an appointment please make sure you have your personal information and provider reference information ready & available at time of call.  

What information does a client need when he calls /emails to book an appointment ?

1. Full name {1st and last Name on your pictured state license/ID}

2. Contact number & Time to call you back. If you & I have talked then a time to call and confirm our appt.

3. Name and Contact number of Business or Employer. "If no provider ref" & a time to call

4. Appointment information {day, time & location, {in or out call}

5. Provider Reference: name & contact number, website & email, Date & Location  Provider reference must be verifiable or we will have to do a work reference

6. Date Check/ Preferred411 members please have all your reference information at time of booking. 
7. Any other information you think will help to speed up the verification process or information on the type of appointment you are looking for.
